New Prekindergarten and New Kindergarten
Date: April 23rd and 24th
Time: 8:30-12 & 1:00-3:00
Following documents MUST be presented:
Child's Certified Birth Certificate
Child's Social Security Card
Child's UP TO DATE Shot Record
Parent's Valid ID
Proof of Residency (bill with parent's name and address)
Copy of Guardianship Paperwork if the child is not living with parents
Child's Medicaid or Insurance Card (for Prek students only)

Please send a complete change of extra clothes for your child - underwear, socks, pants/shorts and shirts in a bag with your child's name on it. We do not want to inconvenience you at work or at home with a phone call to bring your child an extra set of clothes due to an unforeseen accident.
